Application for Services - Early childhood, daycare, therapy

This application begins the process for your child to be enrolled at Sunshine Center or Ready Set Grow (RSG).  Please print the form and return it to Alison Baker at Sunshine Center when it is complete.

Daily Sheet - preschool

The daily information sheet is to be filled out and placed in your child's classroom each morning. In the afternoon or evening when you pick you child up it will have a record of their daily activities.

Illness Policy - all programs

Guidelines established by Sunshine Center and RSG in regard to procedures to be followed when your child is ill.

Lunch Service Schedule

Breakfast and lunch times for all classrooms at Sunshine Center, your child will need to be in their classroom by the prescribed time if they are to have breakfast or lunch served to them.

Medical Statement for Student Requiring Special Meals

This form is for students with special dietary concerns because of illness or allergy.  Please consult your physician.

Prescription Form for Medications and Medical Procedures

No medication will be given at Sunshine Center School or RSG without a physician’s order, including breathing treatments and tube feedings.

Physician's Consent for OTC Medications

No medication, including over the counter medications, will be given at Sunshine Center School or RSG without a physician’s order.

Physical Form

Physical and immunization form must be filled out and returned to Sunshine Center or RSG in your child’s first month at the school and will need to be repeated every 12 months.

Parental Medication Consent Form

If your child needs to take medication during the school day, we ask that you complete this consent form and (if possible) bring the medicine and this form to the nurse or teacher at Sunshine Center School.

News
